Appointment of Enduring Guardian
An Enduring Guardianship is a legal arrangement governed by the Guardianship Act 1987 (NSW).
You, the appointor, appoint another person, called the guardian, to make decisions for you about your health and lifestyle if you lose the capacity to make these decisions for yourself.
An enduring guardian can make decisions about your:
Healthcare: decisions regarding what medical treatment, surgeries, or other health-related matters you receive.
Accommodation: decision about where you live: for example, you may need assistance in your day-to-day living. This may entail staying at home with professional support or moving into an aged care facility.
Lifestyle Choices: decisions about which personal services you receive. This may entail dietary preferences, recreational activities, or care arrangements.
Medical Consent: your attorney can provide or withhold consent for medical procedures.

Revocation of a guardianship can be done provided you have the legal capacity to do so. This entails completion of a form prescribed in Schedule 2 of the Guardianship Regulation 2000.
